SOAP Layout
SOAPS have the following layout:

S - Subjective: Observations and pet history notes usually gathered before or at the beginning of the exam.

O - Objective: Findings by the veterinarian during the physical exam.

A - Assessment: Diagnosis and Differential Diagnosis.
-
P - Plan: Services provided and instruction, notes and recommendations for the pet parent.

It is very important to add any allergies that are learned about during the exam (either from a reaction or from the owner) to the SOAP. This can be done by clicking on the "Allergies" widget to the right of the "S" or Subjective box.

If any diagnostics are performed in-house, these should be noted in the "P" or Plan section, as well as added to the "Labs/Tests" widget to the right of the "O" or Objective box. External labs that are sent out will be added into the system when the results are returned to the clinic. The service should still be noted in the "P" or Plan section in the meantime.

Add any medications provided at the clinic to the medications widget to the right of the "P" or Plan section. Make sure the dosage and instructions are correct.
