Each pet visit must have medical records or a SOAP to record the owner's concerns, what services were provided, the doctor's notes, and future recommendations.

Creating a New SOAP
To create a new SOAP, select the "Charts" option on the "Choose Action" page.
A widget will pop up showing previous charts, select the "plus" icon button. Then select from the drop down items.
Adding Pre-Visit SOAP Details
Write out the reason for the visit and planned services to be provided during the visit into the "CC" box. Keep in mind, these may change during the visit and need to be adjusted.
Add any concerns or observation from the owner into the "S" or Subjective box.
Add all services given, and instructions and recommendations from the veterinarian into the "P" or Plan box. Also write out medications and dosage instructions here. Then, add medications to the "Medications" widget on the right side and adjust dosages to match.
