When a new pet becomes a client at the clinic, VAs need to ask for previous medical records or get verbal pet info from the pet's owner. The basic pet information needed includes:

Name
Birthdate
Sex
Breed
Allergies & Reactions
Previous Health Concerns or Diagnosis
Previous Diagnostics
Reason for visit

When is each provided service recommended again for that pet?

Vaccine History

VAs need to collect and review previous vaccine records (if available) in order to determine when next vaccines are due.

Dogs:

Rabies
Distemper (DHPP) *
Leptospirosis *
Bordetella
Canine Influenza (CIV)
